Financial Policies
Berkley Medical Center strives to provide high-quality care. It is each patient’s responsibility to meet all financial obligations. Each patient is responsible for contacting their insurance company for questions regarding co-pays, deductibles, and covered benefits.
Appointment Policies
Patients are responsible for informing the office of any appointment cancellations. We ask that you contact the office at least 24 hours in advance to inform us of any cancellations. Walk-in visits are available but are subject to extended wait times. We encourage you to make an appointment.
Prescription Refills
Prescription refills will be completed within 48 hours of the request. If calling to request a refill, please have the name of the medication, dosage, and pharmacy information ready to provide to the staff.
Paperwork/Forms
Paperwork/forms will be completed within 2 weeks. The office will contact the patient when it is ready to be picked up.
Referrals
Patients must request all referrals required through insurance at least 2 weeks in advance. The office cannot guarantee that referrals will be completed on time if not requested in advance.
