Form i-693: Immigration Medical Examination

Dr. Misho is a USCIS designated civil surgeon and is certified to perform the immigration physical examination. She is familiar with USCIS requirements and has trained staff members to work with patients seeking this exam to simplify the process.

The examination includes the following:

• Physical exam by Dr. Misho, a USCIS designated physician

• Required laboratory examinations

• Discussion regarding required vaccinations and prescriptions to receive vaccinations

• A sealed, completed i-693 form and a copy for the patient’s records

Requirements for the i-693 form are set by the USCIS. Dr. Misho complies with all updated guidelines and requirements.
